A 54-year-old man presents with slightly blurred vision and polyphagia that has developed over the past 6 weeks. His height is 5 ft 7 in and his weight is 193 lb (BMI 30.2). The patient does not have a regular exercise regimen. History is significant for hypertension and sleep apnea, and family history is significant for maternal colon cancer and paternal type 2 diabetes. Physical examination reveals a hemoglobin level of 7.6 mmol/L.
