Prevention of ovarian remnant syndrome: Adnexal adhesiolysis from the pelvic sidewall

In this video, the authors demonstrate the surgical techniques necessary to remove an endometrioma that is densely adherent to the pelvic sidewall to avoid formation of an ovarian remnant. The key methods highlighted during adhesiolysis focus on the devascularization of the ovary, retroperitoneal dissection, ureterolysis, and mobilization of the rectum to achieve adequate tissue margins around the adherent ovary. Utilization of these critical steps will ensure complete and safe removal of the adnexa, the best strategy in the prevention of ovarian remnant syndrome.
