Morcellation at the time of vaginal hysterectomy

Brought to you by the Society of Gynecologic Surgeons. In this video, the authors demonstrate techniques for performing morcellation at the time of vaginal hysterectomy in properly selected patients. Techniques include intramyometrial coring, anterior wedge resection, myomectomy, and posterior wedge resection. Important concepts to remember include the need for complete devascularization prior to morcellation, adequate visualization, and correct orientation of the scalpel toward the midline to avoid uterine perforation and injury to surrounding structures. A bayonet-shaped scalpel may be used to allow the cutting blade to be better visualized.
