A 15-year-old girl presents with fatigue, dyspnea, intermittent wheezing, and persistent cough. She has experienced three bouts of pneumonia — the last one 6 weeks ago — within the past 7 months that have been refractory to antibiotics. She has a history of asthma that has been moderately controlled with albuterol (ProAir inhaler) and fluticasone (Flovent). Auscultation reveals diffuse rhonchi. The patient appears fatigued and has lost 7 lb since her last follow-up exam 3 months ago. She is also complaining of soreness in her chest.
