This May Be Underrecognized in Allogenic HSCT Patients

Pulmonary manifestations of chronic graft versus host disease (GVHD) were under-recognized in people receiving allogeneic hematopoietic stem cell transplantation (HSCT) in a study involving 35 individuals.

Investigators analyzed autopsy results of patients who underwent allogeneic HSCT at a single institution and died of any cause. A pathologist who did not know the autopsy results looked for presence of pulmonary GVHD.

Airway disease, vascular disease, and interstitial disease were all clinically under-recognized, compared with pathological findings. Varying degrees of pathological changes were detected as follows:

  • ~3 in every 10 patients had bronchiolitis obliterans.
  • ~One-third had pulmonary veno-occlusive disease.
