Questions 1 and 2 are based on the following case:

A 90-year-old woman is referred to your office after an elevated total protein level of 8.3 g/dL is found on routine blood testing. Serum calcium is 9.2 mg/dL, hemoglobin is 12.3 g/dL, and serum creatinine is 0.96 mg/dL. Radiologic testing shows no lytic skeletal lesions. Serum protein electrophoresis and immunofixation show an IgG-kappa M protein level of 0.2 g/dL. A 24-hour urine collection is negative for protein. Bone marrow biopsy shows 6% plasma cells (normal range 0–3%). She feels well and otherwise reports no symptoms.
