Hypertension, white matter hyperintensities, and dementia: What’s the link?

Key clinical point: Uncontrolled hypertension can harm cognition through vascular pathways independent of amyloid and tau.

Major finding: The effect of uncontrolled hypertension on cognition was seen mediated by white matter hyperintensities load (P = .021) and neurodegeneration (P = .024) but not by biomarkers for amyloid or tau.
